Transcribe patient orders including labs, radiology, other diagnostic tests, medications, etcĮnsure that all clinical data, labs, other diagnostic test results, and the interpretation of the results by the medical provider are recorded accurately in the medical recordĪlert medical provider when chart is incomplete, or if medical record contains errors or inconsistencies documentation in order to reduce errors. Medical Provider-dictated diagnoses, prescriptions and instructions for patient or family members for self-care and follow-up Patient education and explanations of risks and benefits Procedures and treatments performed by other medical providers, including nurses and midlevel providers Patient medical history and physical exam, Strong organizational and customer service skillsĪbility to prioritize work based on volume and deadlinesĪccurately and thoroughly document medical visits and procedures as they are being performed by the medical provider, including but not limited to Strong keyboarding skills to facilitate the transcription of communicationsĪbility to work well in a critical environment and be detail oriented Strong written communication skills, including the ability to listen carefully and accurately transcribe verbal communications on a real-time basis Two years of college, preferably with coursework in medical transcription, medical terminology, and human anatomyĮxperience working in a hospital or clinical environmentĮxperience using an electronic health record systemĪbility to work closely with medical professionals during interactions with patients in a hospital or clinical setting High school diploma or GED, plus two years of post-secondary education Promotes infection control and safety as it relates to the patient and employee environmentsĪccompany the provider into the exam room to document all provider/patient interactionsĪccurately transcribes history, physical examination, laboratory, medication, and radiology notes as dictated by the provider Upholds the patient's rights in the provision of care Performs delegated responsibilities in which education and training has been provided and competency assessed Maintains a safe, secure environment for patients, visitors and co-workers Provides care and performs duties in accordance with the plan of care, unit routine, and hospital policy The Scribe is strictly prohibited from any involvement in direct patient careĭemonstrates knowledge of and competently performs technical procedures Participates in classroom and bedside training/precepting of new hires This includes the status of labs, consults, procedures and re-evaluations of each patient The Scribe assists in the organization of the Physician by maintaining a comprehensive list of active patients while in the ED. The Scribe transcribes the proper diagnosis, discharge or follow-up instructions, and/or prescriptions on to the record as dictated by the Physician The Scribe accompanies the Physician and documents any re-examinations of the patient The Scribe transcribes any consultations or discussions with family and/or the patient's private physician or on call physician or any specialty services The Scribe assists in completing the chart by tracking the status of the results of any lab, x-ray, or other evaluations and will continually check on the progress of this data The Scribe will not transcribe any physician orders (insert physician name)" with the date and time of the entry by the Scribe Such signature must include "(enter name of Scribe) acting as scribe for Dr. The Scribe must sign and date all entries into the medical record, whether electronic or manual, which clearly identifies their role as the Scribe, documenting for the physician. The Scribe transcribes any procedures done by the PhysicianĬollects available patient medication lists for scanning The Scribe accompanies the Physician into the patient examination room in order to transcribe the history and physical exam as stated by the Physician during the interview and examination of the patient
